Personalised Care

Our care team strives to offer excellent care for each resident in the Home. Day and night shifts have allocated unit managers who set up detailed, personalised care plans for each resident on admission. Care needs identified include but are not exclusive to mobility, feeding, hygiene, dressing, mood and social needs. These plans are updated following an internationally standardised assessment (interRAI) conducted by our occupational therapists two weeks after admission and reviewed every 3 to 6 months, depending on need. Additionally, our interdisciplinary team meets monthly to discuss the changing needs of our residents. Assistive devices such as hoists, commodes, bath beds, and other transfer aids are used upon recommendation by our occupational therapists. Our carer-to-resident ratio is approximately 1:6, with a ratio of 1:4 in our Memory Care Unity (MCU).

Specialised Memory Care

In our MCU, we ensure that our residents are out of bed during the day, spending time in the supervised communal lounges for meals and activities. The MCU comprises 3 ‘pods’ in which we provide individualised care for each resident based on their unique needs. Carers are guided with setting out residents’ full daily schedules to create structure and provide ongoing companionship. Through the adaptation of daily activities to meet the functional ability of our residents, we strive to preserve and prolong their independence and quality of life.
