In this pack, you’ll find the following forms;
1) Personal Details
2) Medical Report (print and complete with your GP)
3) Montreal Cognitive Assessment (print and complete with your GP)
4a) Applicant Financial Assessment (full disclosure as required by the Department of Social Development)
4b) Family Financial Assessment (children to complete where a fee subsidy is sought)
5) Consent
6) Advanced Directive
7) Service Level Agreement
8) Accommodation Guidelines (just FYI)
Please complete our forms in full and return them to our Social Work team: socialwork (@) highlandshouse (dot) co (dot) za
Along with the above, please submit:
a) Photo/ colour copy of applicant’s ID (front and back if it’s a card)
b) Medical aid details, if applicable
c) Pension details, if applicable
d) Power of Attorney/ Health Proxy documents, if applicable
