Social Wellness
Residents begin their journey at Highlands House with the Social Work team facilitating a full psychosocial and functional assessment of all applicants. The interdisciplinary healthcare team then collaborates to determine eligibility, the level of care needed, and suitable placement. On admission, a care plan is devised to address the resident’s needs, which is regularly evaluated.
We understand adjusting to a care setting can be challenging for older persons who are used to independent living. We help new residents orientate themselves to the Home and encourage participation in Lifestyle Centre activities. We provide regular information, education, support and feedback around socialisation, conflict resolution, and life in a communal setting. We aim to partner with family and close friends to ensure continued and inclusive support. Residents can regularly report on their needs, which enables our staff’s responsive care.
We are here to help. We are experienced in coping with chronic pain, multiple losses, physical limitations and mental health concerns. Our social workers assist by offering individual and group counselling as needed to overcome emotional, psychological and social obstacles to adjustment. We offer support with health decline, terminal illness, and grief. Our Compassion Circles are open to residents and families experiencing loss. We also assist with appropriate referrals to expert professionals and resources where needed.
